CCHI’s Best Practice Series is designed to be relevant to the current issues facing our members and assist them in local and statewide outreach and enrollment. These materials are provided as a resource for use by our members. 2-1-1 San Diego
On June 13th, 2016, 2-1-1 San Diego showed us its state of the art resource and information hub that connects people with community, health and disaster services through a free, 24/7 stigma-free confidential phone service and searchable online database. 2-1-1 serves the entire population of the County. 

Patient Navigator Programs
On November 9th, 2015, Sacramento Covered graciously presented us with an explanation of their highly successful Patient Navigator Program. Through this program Sacramento Covered helps consumers understand and properly utilize their insurance, staying healthy and out of the emergency room. Their work is funded by insurance plans wishing to minimize unnecessary usage of the Emergency Room, which can be costly and divert resources from people with critical emergencies.

Patient Navigator Programs can be a great way for outreach and enrollment organizations to partner with hospitals and health plans to improve patient outcomes while generating sustainable revenues for the organization.

Training presented by theCalifornia Welfare Directors Associationdesigned to address Health Care Reform (HCR) coverage requirements, eligibility programs under HCR, applications and application processing and discuss key eligibility requirements.
 

In the Mix: Preparing to be Assisters

Training that covers current information about the role of Certified Enrollment Entities, Certified Enrollment Counselors and Navigators in California and what it means for CCHI members.
 

Making Health Reform Work at the Local Level

Designed to educate members of recent ACA policy changes at the federal and state level, how the role of CCHI members intersects with these health reform     changes, and how to best prepare for implementation.
 

Premium Tax Credit Counseling

Training on what premium tax credits mean for Californians, how they work, the challenges in counseling families through the process and how members can   meet these challenges and be advocates for eligible families. This session was presented by Health Care for America Now.
 

Overcoming Barriers to Access to Health Care by Immigrant Families

Solution-based presentation that addresses common barriers and how our members can best assist these hard to reach populations in accessing coverage  and services. It offers strategies and tools to meet the needs of these communities and build on the trusted relationships our members already have with them. This session was presented by the National Immigration Law Center.
 

From Innovation to Sustainability: A Program’s Perspective on Successes and Challenges of Implementing a CHW Program in an HMO Setting

The Inland Empire Health Plan presented on a pilot program designed to decrease unnecessary use or avoidable visits to Emergency Department services through outreach and education to families about alternative services through culturally and linguistically appropriate community outreach, including home visits and program linkage.
 

Medi-Cal Administrative Activities (MAA) Training

A presentation to help members learn more about the MAA program, what activities are reimbursable through MAA, what the program requirements are for participating, and a simple formula to help you figure out members’ potential MAA reimbursement.

Sub-Contractor COVID-19 Disclosure

While contracting with CCHI to provide Outreach, Enrollment, Retention, and Utilization (OERU) services to your local communities during this COVID-19 Pandemic all staff/promotoras should follow local health guidelines and procedures to reduce risks of exposures to COVID-19.

CCHI understands that the nature of some outreach and enrollment service activities may require in-person interactions and the risks for providing in-person services during this pandemic. CCHI urges Subcontractors to come up with other avenues of providing contactless services such as utilization of communication platforms to provide services. If virtual services are not possible, all staff/promotoras must follow local guidelines.

CCHI will not be liable for staff/promotoras of Subcontractors if exposure to COVID-19 were to happen. If you or your staff/promotora need training or assistance strategizing ways of providing OERU virtual services please let the CCHI Team know.
